Voluntary recall of Apo-Warfarin 1mg tablet (Batch no: JL4994)

Pharmaforte updates healthcare professionals on the recent recall of their Apo-Warfarin 1mg tablet (Batch No: JL4994) due to deviation in the product's quality specifications. The INR of patients taking this affected batch of Apo-Warfarin may be affected by this deviation in the product's quality specifications. Prescribers whose patients that have been prescribed Apo-Warfarin 1mg (Batch No: JL4994) are advised to initiate additional monitoring processes. Additionally, prescribers are also advised to monitor their patients closely if patients on the affected batch are substituted with a new unaffected batch of warfarin.

Step 1: Scroll down to the bottom navigation panel of the HPP web page. Under the Healthcare Professionals section, expand the dropdown option for ‘Healthcare Professional Portal’ and select your profession. You will be redirected to the specific HPP web page to log in.

Step 2: Click on the Restricted Content icon on the left panel of the web page and log in via SingPass.

Step 3: Click on the MOH Alert icon under the ‘Applications’ section.

Step 4: Click on the Drug Alert tab to view the DHCPLs.
